. . . a model of care that is family-oriented and community-based
. . . with an emphasis on the strengths that each child,
family and community bring to supporting its members
Strong Families, Strong Communities

TriCounty CMO, a non-profit care management organization for children with complex emotional and behavioral issues, understands that the family matters. Family empowerment is a central objective of our mission as evidenced by our “one family, one plan” philosophy within which family-specific plans are developed at the community level, incorporating family needs along with any system mandates. With measurable outcomes identified and the understanding that flexibility is key to positive outcomes, progress is monitored and changes are made to the plan accordingly.

We strive to ensure that our children have access to tailored, comprehensive formal and informal resources. In addition to clinical, educational, vocational and residential services, we encourage partnership with community-based informal resource providers in fostering a child’s strengths, interests and direction towards positive change.
