TCCMO Provider Information


How to become part of the Tri-County CMO Provider Network


All agencies and community organizations are welcome to be part of the network of care for Tri-County CMO families and to participate in planning services, such as clinical, educational, vocational and residential or resources such as support groups, mentoring programs and recreational programs that contribute to a family's resiliency and stability in the community.

The first step in becoming part of our network of providers is learning about the roles and responsibilities of both Tri-County CMO and your organization in a partnership.  Our Provider Agreement defines the roles and responsibilities of each partner and seeks to enhance the quality of services for families while strengthening the relationship between providers and the community.

The second step is to contact us.

Organizations can also choose to become part of our Provider Directory independent of whether they choose to be a network partner. Service criteria and contact information is also entered into our Resource Database. This database directory acts as a resource to our care managers, families and system of care partners who wish to locate services in their respective communities.

If your organization is interested in being listed in our Provider Directory, please download the Community Provider Form (24.60kb) . Once completed, you may either mail the form to our street address or email the same to info@tricountycmo.org.
